CHOICES Doctor Visit Checklist
A worksheet to fill in before an appointment, so you can focus the visit and leave with a plan.
- Free
Free for everyone. There is nothing to buy.
Who this is for
For anyone dealing with symptoms after cancer treatment who is not getting traction in medical visits.
Symptoms can include hot flashes, sleep disruption, vaginal dryness, pain with sex, urinary symptoms, mood changes and joint pain.
What this checklist does
The short version, taken from the guide itself.
- It is a worksheet with a 60-second summary, a 0 to 10 symptom tracker, what matters most to you, questions to ask, and a place to write the plan you and your clinician agree on.
- The first six pages are the essentials. The pages after add your full history and are optional.
- You do not have to complete every section.
What it does not say
- It is not a treatment plan or medical advice. It is a tool to support shared decision-making with your healthcare provider.
One question to bring to your doctor
“If you’re not comfortable managing this, who can you refer me to who is?”
A short sample
“If you are dealing with symptoms after cancer treatment and you are not getting traction in medical visits, this checklist is for you. You do not need to be “bad enough” to use it. If symptoms are affecting your life, they count.”
From the free Substack post that introduces the checklist.
